    A: To make the most of your appointment and ensure a seamless experience, please bring:
    •All school or sports forms that require completion by our providers. Please fill out your sections beforehand.
    •Your child's most up-to-date immunization records, especially if you are a new patient joining our Little Oaks family.

Please allow 3 business days for us to complete standard school and sports forms. Please note that if you bring the form to your child’s physical or well-check appointment, your pediatric provider will complete it while you’re in the office.

    A: Forms such as FMLA (Family and Medical Leave Act) and EFMP (Exceptional Family Member Program) often require a more detailed review and medical documentation by our pediatric providers. For these forms, please expect a longer processing time of 5 business days.

    Q: Can I drop off the forms and pick them up later?
    A: Yes, you can drop off the forms at our front desk during our regular business hours. This helps us manage the workflow efficiently. Please ensure that all necessary sections requiring your input are completed before dropping them off. We will notify you when they are ready for pick-up, typically via a phone call or text message.
